44 Newbold Road is a CQC registered care home for adults with learning disabilities, Autism and complex needs. We offer people with high support needs an individualised package of support in a positive and stable environment, to build confidence, develop daily living skills and community inclusion based on a person centred assessment with agreed outcomes.

The home is large and spacious set in extensive gardens and is decorated and furnished to a high standard.

Five minute walk into Chesterfield town centre, the service is situated near shops, cafés and local amenities.

In addition to the eight bedrooms the home has a self-contained flat consisting of kitchen, lounge and bedroom with en-suite. This is designed to enable the occupants to develop daily living skills and increased independence.

We currently have one vacancy at the service. As well as a longer term placement we may be able to consider planned respite options.

Focus Support

Potens Focus Support service in Newbury encompasses a domiciliary care and supported living service.

As a service we pride ourselves on nurturing the people we support along a path of progression towards achieving their aspirations and goals whatever these may be. The fully trained staff within the service recognise that independence means something different to everyone and that these nuances are to be celebrated; not seen as barriers.

The supported living service at Queens Road offers placements for up to 8 adults. At this supported community, we strive to provide a service which is as purposeful as it is meaningful; encompassing person led, bespoke support.
The Supported Living service is close to the town centre and community activities, allowing for ease of access. The building is laid out over two floors with 8 bedrooms, all en-suite, a communal kitchen, a lounge and a dining area as well as a conservatory. There is a private and enclosed courtyard garden to the rear of the property.

As a domiciliary care service we provide fully trained staff to support individuals in their own homes. We work with individuals to recognise goals and pathways to achieving these. We recognise the importance of maintaining independence and ensure that this is at the heart of the support provided. The bespoke nature of the support offered by Potens Focus Support promotes a service that evolves with those we support. However, the basis of that support will always be consistent… to provide a safe and caring environment where the people we support are at the forefront of all we do.

Good transport links by bus & train to the local community in Newbury and further afield. Selection of local shops within short walking distance. Newbury town centre is less 1/2 mile away with full town amenities: shops / restaurants /Swimming pool/ sports facilities, Parks, Library, medial GP’s, train station, bus/ train station/ Cinema and so on.

Cecil Court

At Cecil Court, people benefit from a vibrant network of staff and friends, while having peace and quiet when they need it. We support adults in developing their life skills, and in doing so, help to nurture their potential to be active members of their community.

The property has 18 one-bedroom apartments which offers an open plan kitchen and living room, a bathroom with fitted shower and a large storage cupboard. People also have access to an accessible garden with a patio area.

Being Active in your community

Cecil Court is in the heart of Langley Moor village close to convenience stores, supermarket, takeaways, hair salon and post office. For people who like to stay active, Meadow field Leisure Centre has a gym, fitness classes, a sports hall and squash & badminton court and a swimming pool. People also like to visit Durham which provides access to Durham Castle & Cathedral, Gala Theatre and Cinema, Shopping Centers and Museums, Durham Market Places, Crook Hall and Gardens.

Achieve with Us

Achieve with Us is based in Leicestershire and provides Day Opportunities for adults with a learning disability, mental health needs and autism. We provide on site and community support for 30 people to access activities and increase their skills and independence.

Day opportunities can give carers or family a break from their caring roles. We can provide a safe, informal, and friendly environment for people, with opportunities to meet others, take part in a wide range of activities, increase life skills and have fun.

OUR FACILITIES AND ACTIVITIES

Achieve with Us have created a safe, friendly and stimulating environment where people will feel comfortable and at ease taking part in activities they enjoy in house or out in the community. Our highly skilled workforce provide professional, experienced and caring support to enable clients to build and live meaningful and independent lives.

Support can be 1:1 or shared depending on the needs of the individual. Our centre operates Mon-Fri between the hours of 9am-4pm. Our facilities include quieter environments for clients who want to take part in activities alone, as well as areas where people can join in with group activities.

We have close links with the Community Learning Disabilities Team, who we access advice and involvement where needed from community nurses, professionals that provide support in all areas associated with adults with learning disabilities and complex needs. We will seek support and advice where needed  for all you support needs.

We are always open to new ideas and our team will work with people to source activities in line with your wishes and goals.

Cannon Street

8 independent flats for adults with mental health needs in the heart of Preston Town Centre, Lancashire.

Offering people with mental health needs an individual package of support in an enabling environment, to build skills, develop confidence and support to achieve their recovery star outcomes.

The service fits into a wider pathway of recovery services for people with mental health issues in Lancashire supported by experienced staff team trained in all aspects of mental health and recovery approaches.

Ideal placement for people moving from residential settings to increase independence in a supportive environment.

The service is built upon our strong track record, extensive experience and approach.

Paxton House

Paxton House is a registered residential care home for up to 8 adults with a learning disability, autism and complex needs.

Paxton House is a large property with excellent facilities located in Borrowash in East Derbyshire. The home takes a person-centred approach and support is organised to focus on agreed outcomes with the people we care for. The home offers individual packages of support facilitating access to, and inclusion within the community.

The service also offers people with high support needs a tailored package of care and support in a stable environment, to build confidence and develop daily living skills.

The home currently has a vacant independence self-contained flat on the second floor.

Cae Glas Specialist Nursing Home

Cae Glas Specialist Nursing Home is an independent nursing care home registered with Care Inspectorate Wales.

Our admission criteria is individuals over the age of 18 years who have a primary diagnosis of Mental Illness, and this is likely to be enduring. Cae Glas has the added bonus of having a highly skilled nurse on site 24 hours a day, using their clinical expertise in assessment and leadership of the service.

Our qualified nurses and skilled support workers come with a wealth of experience and are extremely person centred. We believe in supporting people to achieve their outcomes, whether this be gaining voluntary employment, starting a college course to keeping hydrated, showering regularly, having your prescribed medication to joining in fun activities in the home.

We use slow and steady rehabilitation strategies which are always led by the individual in our care. We promote independence whilst getting to know you, your likes and dislikes, and support you to integrate into the local community if you wish. We always have someone available to chat, support or help with your ongoing mental and physical needs, from a walk around Denbigh to just spending time in the lounge together watching a film.

We employ a Recovery Co-ordinator who organises activities including Cymraeg, Crafts, Gardening, Exercise including chair aerobics, Cooking, Pool Tournaments and Aromatherapy to name a few. Each weekend we have a group outing available to visit attractions across North and Mid Wales and the North West of England. Chester Christmas markets, Anglesey fossil museum, beach trips, cinema, castle trips, car boots and arcades are all options to attend.

Cae Glas has a large dining room, two lounges each with a television and games, an activity room and a rehabilitation kitchen where you can make drinks or snacks whenever you wish. Meals are also prepared by our trained Head Chef who caters to individual needs and always delivers the best parties. We are very proud of our garden which is a lovely peaceful area to spend your time.  There is a large space with garden furniture and a covered area for shelter when required. The greenhouse provides an opportunity to grow plants and vegetables. In the warmer months we have barbecues and parties outside and in the colder ones we have been known to host visits from a choir and some baby bunnies. A smoking area is also located in the garden for those who require it, as is smoking cessation advice and support.

Denbigh is a great location, having the benefit of being semi-rural but within easy access of neighbouring larger towns such as Rhyl and Llandudno. In Denbigh there are several shops, food establishments including butchers, a college, leisure centre, library, theatre, nail salon and a post office. We also have the beautiful Denbigh Castle and many local walking areas available to visit.

North Wales Supported Living

Potens’ North Wales Supported Living (NWSL) Services provide CSSIW registered domiciliary care & supported living services across North Wales.  Providing personalised, specialist support to individuals living in their own homes and person centered support packages for individuals with learning disabilities, autism, mental health needs and other complex needs.

The service:

  • encourages positive community integration through accessing local activities and amenities;
  • provides support that empowers the individual to achieve their own goals;
  • focuses on the development of skills to maximise independent living;
  • with access and guidance in assistive technology where possible, with support towards managing individual budgets.

Park View

Parkview is a 9 bed residential service located in Salford facing Buile Hill park. We provide support for adults with a range of Mental Health issues requiring support to live as independently as possible.

Parkview is a large spacious detached house with spacious communal areas. All communal areas can be accessed by residents along with their relatives and friends.

Parkview has staff on site 24/7 where we will help you focus and develop or re-develop your living skills.

We promote the use of the Recovery star for those recovering from a Mental Health illness. Together you will work through ten areas of your life. This is a good and positive way to monitor your recovery and skills gained whilst recovering.

Primavera House

Younger Adults Transitions Supported Living Service

We currently have a ground floor accessible room with wetroom ensuite and a first floor ensuite room available.

Primavera House is a lovely large detached house which is based in Hampshire on the border of Surrey. The supported living service supports 6 young adults with learning disabilities to live as independently as possible in their local community. Each person has their own room, with en-suite, as well as access to communal areas. The staff team are also skilled in managing complex epilepsy and physical disabilities.

This is a transitional service that supports young adults in developing their life skills, and in doing so, helps to nurture their potential and to be active members of their community.

We work in partnership with schools & colleges so that the people we support, continue to achieve their educational potential. We believe that everyone who resides at Primavera House should be positively challenged to maximise their potential. The outcomes that can be achieved are endless.

The property is owned and managed by a private Landlord, and Potens staff provide the care and support to individuals who live at Primavera, through the Domiciliary Care Agency Surrey.

Regulation and registration

Primavera is a supported living service is registered through our local Domiciliary Care Agency to provide personal care with CQC and has been registered since March 2016.

Services we provide:

We provide support and care as directed by a need led assessment to younger adults with learning disabilities, mental health needs, and physical disabilities in their own homes and in the community.

The manager and staff will work with individuals others that are significant in the individuals life to help design the care and support plans to meet the individual’s needs.

Each person will own their care and support plan and this will be tailored to the individual to ensure inclusion and understanding. A copy of the care and support plan will provided to the individual and a copy retained in the agency office.

Outcomes we are committed to achieve:

  • We are committed to delivering quality care younger adults in their own home.
  • We will listen and react positively to individual needs and wishes therefore will not discriminate due to personal attributes.
  • We are committed to ensuring that individual’s rights are respected and protected supported to meet their individual goals and aspirations, through person centred support.
  • We are aware that all staff are guests in People’s homes therefore we will ensure that our staff respect service users homes and that the policy of respect is adhered to at all times.

We will ensure that the staff employed by the agency, are able to meet the personal, cultural, religious and specific needs of the person being supported. We understand and respect the right of the individual to make choices on who they wish to be supported by.

Communication

People communicate in many different ways and we need to make sure that we ensure that’s everyone’s way of communicating is recognised and valued.
All staff will undertake a range of practical needs led training to enable them to meet the individualised needs of the younger adults who they will be supporting. Examples of communication tool and methods include:

  • Makaton ( based on British sign language)
  • PECS (picture exchange communication system)
  • Easy read symbols
  • Picture boards
  • SLT (speech & language therapy).

Behavioural support and specialist training

Potens employs a regional Positive Behaviour Support (PBS) Facilitator who works alongside our operational team to support managing children, young people and adults with complex social and communication needs. Additionally, Potens staff complete in-house PBS training which ensures that have the knowledge and confidence to work with individuals complex needs..

Referrals

We welcome referrals from all stakeholders and visits to the service. Parents and young people are encouraged to visit the service, however any formal referrals need to be signposted through the designated local authority social worker or care manager.
