Oakleigh

Providing specialised residential care for adults aged 18- 64 and 65 years and over (male/female) that have learning difficulties with some behaviours perceived to be challenging. The purpose of the service is to help a maximum of 11 adults between the ages of 18-65 with learning difficulties achieve the quality of life that most people would want for themselves. With this in mind we believe that the purpose of the service should be to provide a surrounding that is both homely and understanding. We believe that self respect and confidence is important and if we are to achieve this purpose then each person needs to feel they belong.

Oakleigh provides residential accommodation for up to 11 people within the seaside town of Dawlish. The main house is a large spacious Victorian dwelling with an extension adjoining the kitchen. We provide multi-purpose rooms equipped for a variety of indoor activities including Darts, Pool, Table football, Table Hockey, X-Box game play, Karaoke, Crafts, Computer games and DVD’s. Our extension also comprises of a gymnasium to enhance fitness, health and well being. Bedrooms are all good sizes and provide space and opportunity for movement and visitors. Each room has a colour TV, and musical equipment is encouraged. Oakleigh Mews has three apartments providing a lounge, kitchen, bedroom and wash room, all of which form comfortable size living quarters for each individual. The service has over 3 and a half acres of land where the people we support rear Chickens, Ducks, Sheep, Turkeys and Geese as well grow organic fruit and vegetable to promote a healthy diet.

The service is focused on developing the levels of independence with an outcome of being able to move on to supported living wherever possible. We have also been able to enable people to develop the skills to gain paid part time employment as part of their journey. In both cases we can demonstrate a track record of achieving both outcomes.

Supported Lives and Skills4Life Centre is based in Bradford providing community and supported living to adults with a range of needs including learning disabilities, autism, physical disabilities and mental health needs.

We are registered with the Care Quality Commission (CQC) who regulate care services in England.Supported Lives offers supported living, domiciliary, respite and person-centered social inclusion support.

Our linked Skills4Life Centre supports people to develop their skills through curriculum-based sessions and activities in house and in the community.

The Good Days Project

A bespoke support service for people with autism, profound multiple learning disability and complex needs.

Our highly skilled staff team provide a range of support options including 1:1 support, community activities, day opportunities, support in the community and support at home. We design our service around individual need with the key objective of supporting people to reach their full potential and maximise their enjoyment of life.

The Project has an inclusive approach to accessing the community. With a careful planning process, that fully takes in to consideration each person’s needs, we can match activity packages and delivery to individual’s wishes.

We have hubs located throughout Sheffield offering fully accessible bases where personalised activities can be enjoyed. These environments are designed with fully accessible facilities including wet room and wheelchair accessible kitchen, to enable people to benefit from a safe space to enjoy activities whilst receiving the required support.

We also have a number of transport options including wheelchair accessible transport, allow us to design a truly bespoke package of support.

Larwood House

Larwood House is a registered residential care home for up to 9 adults with a learning disability and complex needs.

The home is a fully accessible bungalow with gardens and patio area in Kirkby in Ashfield. Taking a person centred approach, support is organised to focus on agreed outcomes.

Well trained and value driven staff team that support an effective communication system and purposeful pathway. Individual packages of support facilitating access to, and inclusion within their community.

The service offers people with high support needs an individualised package of care and support in a stable environment, to build confidence, develop daily living skills and support to achieve outcomes.

Support is delivered by a skilled staff team with a minimum of NVQ level 2; learning disability & positive behavioural support accreditation; and training & guidance from our Behavioural Support & Therapy team.

Rochdale Dom Care

Registered domiciliary care & support service for the Rochdale and surrounding areas (Bury, Whitefield, Bacup).

Our local staff team provide outreach, community and specialist  support to people across the region in their own homes.

This registration also includes delivering support within  a 4 bed supported living service and soon to be launched 10 self-contained flats within a newly developed skills centre.

 

Reach Skills Centre Blackburn

REACH Skills Centre is based at our Olive Tree Domiciliary Services offices offering a service for individuals to build and develop their skill range through curriculum-based sessions and activities in house and out in the community.

The students in REACH work closely with the ASDAN programme. ASDAN is an education charity and awarding organisation providing courses, accredited curriculum programmes and regulated qualifications to engage, elevate and empower individuals.

Support can be 1:1 or shared depending on the needs of the individual. REACH skills centre operates Mon-Fri between the hours of 9am-4pm. All REACH staff are fully trained in line with national requirements and have a wealth of experience in the care sector. REACH staff have a shared vision focussed on promoting independence, choice, individuality and work towards achieving outcomes and goals.

Olive Tree Domiciliary Services

Olive Tree and Reach Skills Centre are based in Blackburn providing community support to adults with a range of needs including learning disabilities, autism, physical disabilities, and mental health needs.

Olive Tree offers supported living and domiciliary care serviced to people across Blackburn and our linked Reach Skills Centre supports people to develop their skill range through curriculum-based sessions and activities in house and out in the community.

Olive Tree is a support service for adults with learning disabilities, autism, mental health and complex needs. We are registered with the Care Quality Commission who regulate care services in England.

Olive Tree offers individual, person-centred support packages tailored to the individuals needs. This support can be offered in either a supported living arrangement or through outreach support. Support packages can start from 15 hours each week to 24 hours support daily.

All Olive Tree staff are fully trained in line with national requirements and have a wealth of experience in the care sector. Olive Tree staff have a shared vision focussed on promoting independence, choice, individuality and work towards achieving outcomes and goals.

Pendle View

Pendle View is a residential service providing accommodation & intensive support for adults with mental health needs. The service is located in the heart of East Lancashire within the town of Nelson.

Our team supports people with the skills to enable them to take control, increase their independence and reduce support overtime in line with their need.

We have many examples of individuals moving onto supported living/own tenancy as part of a pathway to independent living.

WE SUPPORT A RANGE OF INDIVIDUALS:

  • Mental health needs
  • Dual Diagnosis: personality disorder, bi-polar, schizophrenia, depression, poor motivation, self harm.
  • Age 18+ Male and Female

Support is offered to encourage independence & crisis management within a safe homely environment. The service provides a person centred, recovery focussed approach to achieving goals and positive outcomes.

Accessing appropriate support and liaison with local professionals and organisations focused around the ten outcomes of the Mental Health Recovery Star.  Support Staff work in partnership with external professionals to involve residents with local therapeutic resources such as CPN’s, mental health clinics and substance misuse teams.

East Lancs Dom Care

Registered domiciliary care & support service for the East Lancashire  and surrounding areas. Providing personalised, specialist support to individuals living in their own homes and person centered support packages for individuals with learning disabilities, early onset dementia or mental health needs.

Encourages positive community integration through accessing local activities and amenities. Provides support that empowers the individual to achieve their own goals.

Focuses on the development of skills to maximise independent living, with access and guidance in assistive technology where possible, with support towards managing individual budgets

Supported Living service offers Tenancies and support for four people with learning disabilities and complex needs. Encourages a homely environment, with a variety of facilities. We take a person centred approach, encouraging independence focusing on outcomes of individuals. We Support effective communication & encourage activities of daily living.

5 person detached house in semi-rural village, 2 miles east of Ormskirk Town Centre. Communal kitchen/diner,  lounge, utility room and 2 x upstairs bathroom / toilet  (one with shower & one with bath) plus ground-floor 1 x wet room / toilet.
