Orme House

Orme House is a residential home based on the Orme overlooking the heart of Llandudno. Our aim is to help people regain a meaningful and satisfying life.

We deliver a 24/7 residential services for adults over the age of 18 with severe/enduring and/or acute mental illness. They may also have a learning, sensory or physical disability.

We are committed to ensuring quality outcomes for each person, focusing on recovery and pathways to independent community living.
We support people in managing a range of issues including their finances, diet & nutrition, positive social inclusion and daily living skills.

We encourage a recovery and re-enablement model of support – taking an individualised approach with the aim being to achieve what the person themselves would see as regaining a meaningful life, with staff on hand to facilitate this.

We see social inclusion as an integral part of people’s recovery and encourage community participation in as many opportunities as possible.

Clement House

Clement House is an eight bedroomed residential care home located in the centre of Llandudno.

Clement House has a commitment to promote individual independence by inclusion, involvement and participation. Staff at the service are highly trained to deliver an active support model of care and support to further enable independence as well as in line with the individual needs of residents.

The service is set in a large Victorian semi-detached property close to the town centre and within walking distance of community activities.

All the residents have their own en-suite room furnished to preference, with access to the communal lounges, kitchen and large garden and patio area.

Well trained and value driven staff team that support an effective communication system and puposeful pathway.

Drumary House

 

Registered Residential home, providing accommodation & support for seventeen people with learning disabilities and complex needs.

Takes a person centred approach, focused on positive outcomes for each individual. Encourages a homely environment, supporting daily living activities and facilitating access to local inclusive community activities.

Drumary House is close to the community centre and activities of Drumary & Derrygonnelly. The service offers people with high support needs a package which includes individualised support & care, in a stable environment, aiding activities of daily living.

The skilled staff team benefit from training and guidance from Potens Behavioural Therapy Team, accessing solutions to communication issues, daily living difficulties and any behaviour that challenges.

Facilities – Spacious detached house with 17 rooms, with neutral decoration and space for sleeping & living in each personal room. There are a variety of communal rooms, bathrooms, lounge, dining room & kitchen, plus large gardens to the front and rear of the property.

North West Domiciliary Agency

Potens offers Domiciliary Care services to adults with a learning disability, mental ill health and associated needs across West Lancashire, Leyland and Chorley.

This service is offered to you in your own homes, in the community or in an outreach capacity.

The staff team are experienced in meeting a wide range of needs including behaviour that challenges and complex needs, with support packages individually designed to promote independence and enable you to make your own choice. Supporting individuals to collage, work , inclusion and with all day to day activities.

 

Walmersley Road

Walmersley Road in Bury is a registered residential home for autistic people and people with learning disabilities. Walmersley Road has been accredited with the National Autistic Society (NAS) as providing appropriate support for individuals on the autistic spectrum. The NAS made particular reference to the staff team’s skills in promoting independence, enabling service user involvement and focusing on outcomes.

The staff team are skilled in supporting vulnerable, high functioning individuals with Asperger Syndrome and associated complex needs, such as anxiety, depression, obsessive compulsive disorder (OCD), attention deficit hyperactivity disorder (ADHD), substance abuse and relationship difficulties. Support staff work in partnership with multi-disciplinary personnel to aid effective and appropriate support.

The skilled staff team benefit from working closely with Potens Behavioural Specialist, as well as in-house training and e-learning plus access to Bury PCT training courses.

Large detached property close to local amenities of Bury and transport links into Manchester. The home has six bedrooms with one rehabilitation flat on the ground floor to a provide pathway service.

 

Mansard House

Registered Residential home for adults with learning disabilities, mental health needs including forensic history.

Takes a person centred approach, focused on positive outcomes for each individual. Working within the Mental Health Recovery model, independence, rehabilitation and involvement in local community activities is encouraged.

Facilities – Spacious detached house with 7 en-suite rooms over 2 floors, with neutral decoration and space for sleeping & living in each personal room. There is also a self contained flat with 3 en-suite bedrooms on the top floor to experience independent living.There are a variety of communal rooms, lounge, dining room, conservatory & kitchen, plus a large gardens & patio area.
