Successful Transition from Hospital to Supported Living

 

DB is 21 years’ old and has moved from hospital into his own tenancy at Gilling Crescent, a supported living service in Darlington. With support from his team at Potens, the move has been a big success!

 

D had been in hospital for some time before he was ready to move on and transition back into the community through MTD and work with his family. His social worker had approached Potens and introduced D to the team at Gilling Crescent. Staff visited the ward and spent time with D getting to know him.

 

D also spent time at Gilling Crescent with his new staff team, getting to know the local area, going for walks, and finding out where the shops are and getting a sense of his community. D continued to spend time at the property, staying for meals, eventually progressing to staying overnight. When he felt ready, D was finally discharged and took on his own tenancy agreement.

 

D has said he likes living at Gilling Crescent and has made it his home. The staff are great and spend time with him, supporting and encouraging him to take part in activities and increase his skills. D has said he really enjoys his trips out and has been to Redcar, Richmond and Durham.

 

Since moving in D has also returned to college and attends a walking group once a week. D has made new friends and is successfully living in his own tenancy – with a successful move from hospital back into his local community.
