Potens Surrey Supported Living & Outreach Services

The Domiciliary Care Agency Surrey is part of Potensial Ltd we provide a range of care and support services to adults, young people and children with learning disabilities, autism, mental health, physical disabilities and associated problems.

We support individuals in their homes, on a one to one basis, either for a short period of time, such as getting ready for school, college or for longer sessions completing specific activities such as community support, attending clubs or day to day living. Our service also supports children and young people in educational settings.

Regulation and registration

The Domiciliary Care Agency is registered to provide personal care with CQC and has been registered since March 2016. Our services can be found on the CQC website here.

We support children, young people and adults within the below age ranges:

Children and Young People: +3 to 18

Adults: 18 to 65

Services we provide:

We provide support and care as directed by a need led assessment to adults and children with learning disabilities, mental health needs, and physical disabilities in their own homes, out in the community and at the various clubs we offer.

The agency will work with the children, young people or adults and others that are significant in the individuals life to help design the care and support plans to meet the individual’s needs.

Each Service User will own their care and support plan and this will be tailored to the individual to ensure inclusion and understanding. A copy of the care and support plan will provided to the individual and a copy retained in the agency office.

  • We are committed to delivering quality care to children, young people or adults in their own home, out in the community or attending various clubs that we offer. The agency will listen and react positively to individual needs and wishes therefore will not discriminate due to personal attributes.
  • We are committed to ensuring that children, young people or adult’s rights are respected and protected and that children, young people or adults are supported to meet their individual goals and aspirations.
  • We are aware that staff employed by the agency are guests in Service User’s homes therefore we will ensure that our staff respect service users homes and that the policy of respect is adhered to at all times.

We will ensure that the staff employed by the agency, are able to meet the personal, cultural, religious and specific needs of the person being supported. The agency understands and respects the right of the individual to make choices on who they wish to be supported by.

Communication

People communicate in many different ways and we need to make sure that we ensure that’s everyone’s way of communicating is recognised and valued.

All staff will undertake a range of practical needs led training to enable them to meet the individualised needs of the children, young people or the adults who they will be supporting. Examples of communication tool and methods include:

  • Makaton ( based on British sign language)
  • PECS (picture exchange communication system)
  • Easy read symbols
  • Picture boards
  • SALT (speech & language therapy)

Behavioural support and specialist training

Potens employs a regional Positive Behaviour Support (PBS) Facilitator who works alongside our operational team to support managing children, young people and adults with complex social and communication needs. Additionally Potens staff complete in-house PBS training which ensures that have the knowledge and confidence to work with young people and adults with complex needs.

Staffing training

All staff members will undertake a comprehensive staff induction. We also complete E-learning, on line training which consists of theory based mandatory training courses. NVQs in health & social care are encouraged to expand knowledge and evidence skill and personal development. We also expect a commitment to the Social Care Commitment, which supports and raises workforce quality in adult social care.

All staff will undertake a range of practical needs led training to enable them to meet the individualised needs of the children, young people or the adults who they will be supporting. This allows us to provide an unparalleled level of service to every person whom we support.
